Important Update on Safeguarding Audit Criteria
In response to the recent increase in safeguarding cases, we are currently reviewing the criteria for the weekly safeguarding audit. We have identified certain case types that no longer require clinicians to tick “yes” in the safeguarding box on Adastra, including:
- Unwitnessed falls in nursing homes with no injury, where the patient has returned to their baseline and no concerns are identified in EMIS/Connecting Care.
- Medication management cases:
- Accidental ingestion of medication, with no symptoms or concerns, following a Toxbase review, and no concerns identified in EMIS/Connecting Care.
- Prescription requests where medication has not yet arrived at the pharmacy.
- Failed contacts with no safeguarding concerns identified in EMIS/Connecting Care.
Please note: Children with a history of toxic ingestion must still be referred to the safeguarding audit by ticking “Yes” to the ‘safeguarding concern’ question on Adastra.
If you have a safeguarding concern or have made a safeguarding referral, please remember to tick the PLS box to notify the patient’s own GP.
If you have any questions or need further clarification, please don’t hesitate to contact lynnhaywood@nhs.net or your line manager.

Safeguarding can be wide-ranging, complex, and difficult. Signs can be obvious or much more subtle. Serious safeguarding cases are thankfully rare, but “soft” signs can be the harbinger. All staff must be eternally vigilant.
